Harborlight is a private pay practice. Below is a full breakdown of fees by service, along with our payment and cancellation policies — so you know exactly what to expect before you schedule.

Harborlight is a private pay, cash-based practice

We do not bill insurance, participate in insurance networks, or accept Medicare or Medicaid. All fees are paid directly by the patient at the time of scheduling. HSA and FSA cards are accepted, and superbills can be provided upon request for out-of-network reimbursement.

Fees by service

The Comprehensive ADHD Evaluation and Hormonal Mood & PMDD Evaluation are each billed as a single bundled fee covering all required sessions. Deprescribing is billed using our standard evaluation and follow-up rates below — there is no separate deprescribing fee.

Payment policy

A valid card is kept securely on file for all patients. Payment is due at the time your appointment is scheduled, not at the time of the visit itself — this keeps billing simple and predictable for everyone.

  • A card on file is required to schedule any appointment
  • Payment is processed at the time of scheduling, not at the visit
  • You will receive a receipt for each payment for your records
  • HSA and FSA cards are accepted for all services
  • Superbills can be provided upon request, which you may submit to your insurance for possible out-of-network reimbursement. Reimbursement is not guaranteed and varies by plan.

Cancellation policy

Late cancellation & no-show policy

We ask for at least 48 hours' notice to cancel or reschedule an appointment. Appointments canceled with less than 48 hours' notice, or missed entirely, are subject to a cancellation fee. This policy allows us to offer that time to another patient who may need it.

Common questions about fees

  • Being private pay lets us spend real time with patients, make clinical decisions without an insurance company weighing in, and offer more flexibility in scheduling than a typical in-network practice can. It also means we are not limited by insurance-dictated visit lengths or frequency restrictions.

  • Yes. HSA and FSA cards are accepted for all services at Harborlight.

  • Yes. Superbills can be provided upon request, which you can submit to your insurance company for possible out-of-network reimbursement. Reimbursement amounts and eligibility vary by plan, so we recommend checking with your insurer beforehand.

  • Collecting payment at the time of scheduling simplifies billing and reduces no-shows, which ultimately helps us keep appointment availability open for everyone. You'll always know what you owe before your visit — no surprises after the fact.

  • Rescheduling is easy as long as it's done at least 48 hours before your appointment. If you reschedule within that 48-hour window, the standard cancellation fee applies, the same as a cancellation.

  • No — the ADHD Evaluation and the Hormonal Mood & PMDD Evaluation are each billed as a single flat fee ($825) that covers every session required for that evaluation. You won't see separate per-session charges for these two bundles.

  • QbCheck is included in the bundled ADHD Evaluation fee. It's only billed separately ($225) if you're adding it on outside of a full evaluation, or repeating the assessment at a later date.

  • Deprescribing uses our standard Comprehensive Psychiatric Evaluation rate ($475) for the intake visit, and the standard Follow-Up Visit rate ($250) for each taper monitoring appointment after that. There is no separate or additional deprescribing fee — the cost simply reflects the number of standard visits your taper requires.

  • Yes. As a private pay practice, federal law requires that we provide uninsured and self-pay patients with a Good Faith Estimate of expected charges before treatment begins. You will receive this estimate after scheduling and before your first appointment. If you are ever billed substantially more than your Good Faith Estimate, you have the right to dispute the charge.

Forms & documentation

Harborlight Psychiatric provides certain forms and letters for established patients, at the clinician's discretion and based on clinical appropriateness. Below is a summary of what's available, what isn't, and what each item costs.

  • At the clinician's discretion, we may provide forms such as 504 plan letters, Emotional Support Animal (ESA) letters, brief confirmation letters, and school or workplace accommodation letters. Availability depends on clinical appropriateness and the nature of your treatment relationship — not every form is suitable for every patient or situation.

  • No. Harborlight does not offer long-term disability certification under any circumstances.

  • Short-term disability may be considered for established patients who are participating in a psychiatric day program that requires time off work. This is evaluated at the clinician's discretion and is not guaranteed. An office visit is required, along with the applicable form completion fee below.

  • A brief confirmation letter verifies that a patient is under active treatment and states the length of the treatment relationship. These letters do not include diagnoses, disability determinations, or legal opinions of any kind. One brief confirmation letter is included annually at no charge; additional letters requested within the same year are $50 per letter.

  • ESA letters may be issued when clinically appropriate and after a sufficient duration of treatment, at the clinician's discretion. The flat fee for an ESA letter is $150.

  • If no additional evaluation is required, the fee is $150. If an evaluation is required to support the request, the cost is the applicable office visit fee plus $50 per page for form completion.

  • STD and FMLA paperwork requires an office visit. Form completion is billed at $50 per page, in addition to the office visit fee.

  • Yes. In addition to forms and letters, refill requests made outside of a scheduled appointment are billed at $25. Fees for legal requests are provided upon request — please note that Harborlight Psychiatric does not provide court-mandated evaluations, forensic opinions, custody evaluations, or fitness-for-duty assessments.

  • Medical record requests are processed in accordance with applicable state and federal law. Fees are limited to allowable cost-based charges, and any applicable fees will be disclosed to you before the request is completed.

Forms are available to established patients only

We are not able to complete forms, letters, or documentation for individuals who are not actively established as patients at Harborlight Psychiatric. Whether a specific form is appropriate is determined by the clinician based on the clinical relationship and treatment history.
