Available to established patients only.
Harborlight provides certain forms and letters for established patients, at the clinician's discretion and based on clinical appropriateness. Below is what's available, what isn't, and what each item costs.
What kinds of forms or letters can Harborlight provide?
At the clinician's discretion, we may provide 504 plan letters, Emotional Support Animal (ESA) letters, brief confirmation letters, and school or workplace accommodation letters. Availability depends on clinical appropriateness and the nature of your treatment relationship — not every form fits every situation.
Do you offer long-term disability certification?
No. Harborlight does not provide long-term disability certification under any circumstances.
Can you complete short-term disability paperwork?
Short-term disability may be considered for established patients participating in a psychiatric day program that requires time off work. This is evaluated at the clinician's discretion and isn't guaranteed. An office visit is required, along with the applicable form-completion fee below.
What is a brief confirmation letter, and what does it cost?
A brief confirmation letter verifies that a patient is under active treatment and states the length of the treatment relationship. It doesn't include diagnoses, disability determinations, or legal opinions. One is included annually at no charge; additional letters within the same year are $50 each.
Do you provide Emotional Support Animal (ESA) letters?
ESA letters may be issued when clinically appropriate and after sufficient treatment duration, at the clinician's discretion. Flat fee: $150.
What does a school or workplace accommodation letter cost?
If no additional evaluation is required, the fee is $150. If an evaluation is required to support the request, the cost is the applicable office visit fee plus $150 for form completion.
How is short-term disability (STD) or FMLA paperwork billed?
STD and FMLA paperwork requires an office visit. Form completion is billed at a flat $200, in addition to the office visit fee.
Are there other fees for services outside of scheduled appointments?
In certain circumstances, yes. Fees for legal requests are available on request. Harborlight does not provide court-mandated evaluations, forensic opinions, custody evaluations, or fitness-for-duty assessments.
How are medical records requests handled?
Records requests are processed in accordance with applicable state and federal law. Fees are limited to allowable cost-based charges and will be disclosed before the request is completed.
Forms are available to established patients only
We are not able to complete forms, letters, or documentation for individuals who are not actively established as patients at Harborlight Psychiatric. Whether a specific form is appropriate is determined by the clinician based on the clinical relationship and treatment history.
